Our process

Prior to transition, we’ll work with you to design a placement that is individual to the young person. The 2:1 or 3:1 care team for the placement will be selected with the young person in mind.

We’ll also take the young person’s lead on what they need from us to make the transition more comfortable for them. If that’s hearing the team’s voices, seeing their photos or meeting them before the placement commences then we’ll happily facilitate this.

The transition period is a difficult time for a young person, especially if they have experienced this process multiple times previously. Our teams understand this. They’ve been recruited and trained with this in mind, so will not give up on the young person, no matter how challenging it gets. Instead, they’ll view these challenges through a trauma informed lens, responding with empathy and resilience. Doing so allows our teams to build trust with the young person and reinforce their psychological safety.

Team consistency is key to helping young people feel safe in their placement, so every effort is taken to ensure they are supported by the same people throughout their care. Even when the young person’s permanent team are attending training or taking annual leave, we do not use agency staff, instead our young people will continue to be supported by our internal cover team. Our cover team are permanently employed by Protective Care Group and equally as skilled and specialist in the care we provide.

Through our planned transition care we do more than simply fulfil the day-to-day needs of the young people we care for, we take a wholistic approach to their development. This includes mentoring and coaching young people towards mental, physical, emotional and education regulation.

Our placements are designed to evolve with each young person. We’ll work together, along with the rest of the multi-disciplinary team to adapt them to the young person’s changing needs and goals and progress.

We believe that the service we offer you should be as unique as the care we provide our young people. Our Partnerships Team will work with you to identify what this looks like, but regardless you can feel confident knowing their next steps will lead to a safe, stable placement with positive measurable outcomes.

They’ve been recruited and trained with this in mind, so will not give up on the young person, no matter how challenging it gets. Instead, they’ll view these challenges through a trauma informed lens, responding with empathy and resilience. Doing so allows our teams to build trust with the young person and reinforce their psychological safety.

Our rapid response crisis care is dedicated to returning consistency, stability and regulation back to a young person’s life. Our placements are designed to evolve with each young person. We’ll work together, along with the rest of the multi-disciplinary team to adapt them to the young person’s changing needs and goals and progress.

Once regulated, we often find that a lot of our young people remain with us on a long term basis due to the level of progress they have made and the relationship they have developed with their care teams. Our Partnerships team will be more than happy to review this with you.
